Barber membership request form

A monthly plan turns a regular who comes "whenever" into one who comes every few weeks, but signing someone up at the chair, between two cuts, rarely goes smoothly. This form lets customers ask to join in their own time: which plan they want, how often they'd like to come, which barber they prefer, which days and time of day suit them and when they'd like to start. There is no payment step; a note on the form says you'll confirm the price and how to pay, so you set the membership up the way you already take money.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Rename the plan options after your real plans and put the monthly price in each one, so nobody asks for a plan you don't offer.
  • Put the form link on a card at the mirror and in your Instagram bio, so regulars see it where they already are.
  • Check the preferred barber and days against your schedule before you confirm, so fixed visits really fit.
  • Say in the form description what happens to a missed visit, for example whether it carries over to the next month.

Questions about this template

Why is there no payment on the form?

Because you confirm each membership first: whether the barber has room, which price applies and how the customer pays. After that, take payment the way you do today, in the shop or by bank transfer.

Can members book their visits with this form?

No, it's for joining the plan. For the visits themselves, Zotview's online booking lets members pick a free slot on their own.

Which plans should I offer?

That's up to you, for example a haircut plan, a haircut-and-beard plan and a beard-only plan. The form works with whatever you sell; just rename the options.
