Hair colour consultation form

Colour often goes wrong because of something nobody mentioned: a box dye from last summer, henna, a keratin treatment or a reaction to dye years ago. This form asks about all of it before the client sits down: their natural colour, what has been on their hair recently, the result they'd like, how much upkeep they want and whether they've reacted to hair dye before. With that in hand you can book enough time, plan a strand or skin test where your products call for one, and talk about a realistic result in the consultation instead of at the basin.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Send the link as soon as the colour appointment is booked, so you have the answers before the day.
  • Ask clients to bring or send a few photos of the look they want; for "warm blonde", pictures say more than words.
  • If someone has used henna or box dye, plan time for a strand test before you promise a result.
  • Follow your colour brand's instructions on skin tests; the form records the answer, it doesn't replace the test.

Questions about this template

Why does the form ask about henna and box dyes?

Because earlier colour and some henna products can change how bleach or new colour takes. Knowing about them in advance lets you plan a strand test and set honest expectations.

Why is there a consent checkbox?

A past reaction to hair dye is health information. The checkbox records that the client agrees to their answers being stored and used to prepare for the appointment.

Does the form decide whether a client can have colour?

No. It collects information; you decide, together with the client and following your product's instructions, what's possible and whether a test is needed first.
