Tips for this form
- Rename the plan options after your real plans and put the monthly price in each one, so nobody asks for a plan you don't offer.

- Check the preferred barber and days against your schedule before you confirm, so fixed visits really fit.
- Say in the form description what happens to a missed visit, for example whether it carries over to the next month.
