Lash and brow consent form

Lash and brow treatments happen right next to the eyes, with tints, adhesives and lifting solutions that some people react to. This form puts the questions you'd ask at the desk into writing: which treatments the client is having, whether any listed points apply, such as an allergy to dye or cosmetics, an eye infection or recent eye surgery, and whether they've had a patch test for the product. The client confirms their age, agrees to follow your aftercare advice and types their full name as confirmation, with the date. You keep a dated record for each client instead of a folder of paper.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Send the form with the booking confirmation and ask clients to fill it in before they arrive; it saves time on the bed.
  • Follow your product supplier's patch test instructions, and book the test as its own short appointment before the treatment.
  • If you treat clients under 18, add a field for a parent or guardian's name and have them fill in the form.
  • Ask returning clients to fill the form in again when something has changed, for example a new allergy or eye treatment.

Questions about this template

Is the typed name a legal signature?

It's a typed confirmation, saved with the date and the rest of the answers. Whether that's enough for your records depends on your country and your insurer, so check with them; some salons also ask for a signature on paper at the first visit.

Why ask about a patch test?

Lash and brow tints and lifting solutions are products that some people react to, and many product instructions call for a skin test beforehand. The question shows you at a glance who still needs one.

Why is there a consent checkbox?

The answers include health information, such as allergies or eye conditions. The checkbox records that the client agrees to these details being stored and used to prepare for the appointment.
