Massage intake form

A good massage starts before the client lies down: the therapist needs to know which treatment they came for, how firm they like it, where to focus and where not to touch, and whether anything about their health changes the plan. This form asks exactly that. Health points such as pregnancy, a recent injury or surgery, blood pressure, varicose veins or a skin allergy to oils are ticked from a short list, with room for details, and the client can say whether they'd prefer a female or male therapist. Your team reads it before the session, so the first minutes in the room go to the massage, not to questions.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Send the link with the booking confirmation; first-time clients in particular can answer at home in peace.
  • Go through any ticked health point with the client before you start, and if you're unsure whether a treatment suits them, suggest they check with their doctor first.
  • Name the oils or products you use in the form description, so clients with allergies can tell you in advance.
  • Ask regular clients to update the form when something changes, for example a pregnancy or a new injury.

Questions about this template

How is this different from a spa booking enquiry?

A booking enquiry asks which treatments a guest wants and when. This intake form comes after the booking and prepares the therapist: pressure, focus areas and health points for one client.

Why ask about the therapist's gender?

Some clients feel more comfortable with a female or male therapist, and asking in advance spares them from raising it at the desk. Remove the question if your team can't offer a choice.

Why is there a consent checkbox?

The form asks about health, such as pregnancy or injuries. The checkbox records that the client agrees to these details being stored and used to prepare for the massage.
