Salon appointment request form
Clients request a hair, nail or beauty appointment with their service, preferred day and time of day.
A new client's first visit goes better when you know a few things before they sit down: what they're coming in for, whether they've ever had an allergy to dye or skin products, and whether a patch test is due. This form asks exactly that and nothing more. Allergies are a yes, no or not sure with an optional short note, not a medical questionnaire. The client chooses whether you should book a patch test, agrees that a dye treatment may be moved if the test hasn't been done, and confirms with their typed name and the date.

Zotview for Beauty salonsBecause a salon needs to know whether to take extra care, not a medical history. A short optional note is enough to start the conversation; anything more is better discussed in person.
No. It records whether the client has had one and that they understand a dye treatment may wait for it. The test itself follows your product's instructions.
An allergy answer is health information. The required checkbox records that the client agrees to it being stored and used to prepare their appointment, separately from their typed name.
