Gift voucher request form

Gift vouchers sell in waves around birthdays and holidays, and each one needs the same details: an amount or a specific treat, who it's for, what it should say and how it gets to the buyer. This form collects all of that in one go. The buyer chooses between a value voucher and one for a particular service or product, adds the recipient's name and a short message, and picks collection, post or a printable file by email. There's no payment step: you confirm the request and tell the buyer how to pay, the same way you take payment today.

Tips for this form

  • Name your most popular gifts in the placeholder, for example a set menu for two or a 60-minute massage, so buyers have an idea to start from.
  • If you only sell value vouchers, delete the service field; if you only sell service vouchers, delete the amount, so nobody has to skip a question.
  • Print the validity period on every voucher, and check which periods are allowed where you are.
  • Give each voucher a number and keep it in your records with the buyer's name, so you can check it when it's redeemed.

Questions about this template

Can buyers pay for the voucher through this form?

No. The form is for the request only. After you confirm, you tell the buyer how to pay — in the shop, by bank transfer or however you normally take payment — and send or hand over the voucher once it's paid.

What's the difference between a value voucher and a service voucher?

A value voucher is worth a fixed amount that the recipient can spend on anything you sell. A service voucher is for one specific thing, like a haircut or dinner for two. The form lets the buyer choose; remove the option you don't offer.

Why is the personal message limited in length?

Because it has to fit on the voucher. The field allows up to 300 characters; change the limit in the builder to match your voucher design.
