Vet new patient registration form

At the counter of a vet clinic, registering a new pet means a list of questions while the animal is nervous and the next owner is waiting. This form moves them to before the visit: who the owner is and how to reach them, and the pet's name, species, breed, age, sex, vaccination and microchip status, plus an optional reason for the visit. It asks whether a microchip exists, not its number, and leaves the medical history to the examination.

Tips for this form

  • Link the form in your booking confirmation, so new owners can fill it in at home with the vaccination card at hand.
  • Edit the species list to the animals your clinic treats, for example remove reptiles if you don't see them.
  • Ask owners to bring the vaccination card or pet passport to the first visit, and say so in the form description.
  • Add a paragraph with your emergency number above the form, so nobody uses it for an urgent case.

Questions about this template

Why doesn't the form ask for the microchip number?

Whether a pet is chipped is enough to prepare the visit. You read the number from the chip or the pet passport at the clinic, which avoids typing errors.

Is a pet's health data treated like a person's?

Data protection rules mainly cover the owner's personal data, such as name, phone and email. The form keeps those to what you need and asks for consent to store them.

Does this form complement the human new patient registration form?

It follows the same idea for vet clinics: few questions before the first visit, the details at the examination. Practices for people use the new patient registration form instead.
