Wedding RSVP form

Paper reply cards get lost, and RSVPs by WhatsApp end up spread across three phones. This form asks every guest the same few questions: are they coming, to which parts of the day, with a plus-one or children, which main course they'd like and whether they have allergies or dietary needs. Each answer arrives by email and is saved in one list, so the seating plan and the numbers for the caterer come from the same place. A song request and a note for the couple are optional, for guests who want to add something personal.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Put the reply-by date in the form description and on the invitation. Ask your caterer when they need final numbers and set your date a little earlier.
  • Match the menu options to what your caterer actually serves, and remove the menu question if there is a buffet.
  • If only some guests are invited with a plus-one, say so on their invitation rather than in the form, so nobody feels left out.
  • Export the answers as a CSV file from Submissions and count adults, children and main courses in a spreadsheet for the venue.

Questions about this template

Why is there a consent checkbox?

Allergies and dietary needs can count as health information. The checkbox records that guests agree to these details being stored and used to plan the wedding. Delete the answers once the wedding is over and you no longer need them.

Can guests change their answer later?

Each submission is saved separately with its date. If a guest sends the form again, you'll see both answers and can go by the newer one.

Is this form for couples or for wedding planners?

Both. Couples can send the link to their guests themselves; wedding planners and venues can set up a form for each wedding and share it with the couple's guest list.
