Fitness trial class booking form

A trial class is often the first real contact with a new member, and the coach wants to know who's coming. This form asks which class someone wants to try, how much experience they have and when suits them, plus an optional note about injuries. You reply with a confirmed class time, and a consent checkbox records that they agree to you storing what they've shared.

Tips for this form

  • Replace the class list with your current timetable, and remove classes that aren't suitable for beginners.
  • Keep the coach note optional and ask only what your trainers need to adapt the session.
  • Turn on the auto-responder and use it to say what to bring, such as a towel, water and comfortable shoes.
  • If you'd rather let people pick a free spot themselves, use Zotview's booking system for your classes.

Questions about this template

Is the trial class booked once the form is sent?

No. The form sends you a request, and you confirm the class and time. If you want people to choose from real free spots, use Zotview's booking system instead.

Why is there a consent checkbox?

The coach note can contain health information, such as an injury. The checkbox asks the person to agree that their details are stored and used to prepare for the session. Adjust the wording to your studio and privacy notice.

Can I use it for different classes and locations?

Yes. Edit the class options in the builder, add a question for the location, or use conditional fields to show follow-up questions for specific classes.
