Restaurant table reservation request form
Guests request a table with date, time, party size and any special needs, and you confirm it.
Most guests won't tell you to your face what could have been better, but many will answer a short survey. This one asks whether they dined in, picked up or ordered delivery, lets them rate food, service and atmosphere with stars, and leaves room for what they enjoyed and what you could improve. The email address is optional, so guests can stay anonymous if they prefer.
Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.
