Bridal hair and make-up request form
Wedding mornings are booked long in advance, and one enquiry can mean anything from a single make-up to hair for the bride, her mother and four bridesmaids. This form asks what you need to plan the day and quote for it: the wedding date and the time the bride needs to be ready, whether you work in the salon or on location, which services she'd like, how many others in the party need styling and whether she wants a trial first. Tips for this form
- Work backwards from the ready-by time to see how early you'd need to start; with several people in the party, that decides whether you need a second stylist.
- Say in the form description whether you charge a travel fee for styling on location and how far you travel.
- Offer the trial as a separate appointment once the wedding booking is confirmed.
- Ask the bride to send inspiration photos by message after you reply, so the form stays quick to fill in.
Questions about this template
Why ask for a ready-by time rather than a start time?
Because the ready-by time is what the bride knows, usually from the photographer or the ceremony. You work out the start time from it, depending on how many people you're styling.
Why is there a consent checkbox?
Product sensitivities and skin allergies can count as health information. The checkbox records that the bride agrees to these details being stored and used to prepare for her appointment.
Can I use the form for other occasions?
Yes. Rename the wedding date to the date of the event and the form works for proms, engagement parties, henna nights or photo shoots.
