Catering quote request form
The price of a catering job depends on more than the number of guests. Finger food delivered to an office is a different job from a hot buffet with staff at a wedding. This form asks what you need to price it: the kind of event, the date and time of day, the guest count, the food style, whether it's delivery only, with set-up or with serving staff, whether you need to bring tableware and tables, and where the event is. It also asks how many guests eat vegetarian or vegan and lets the organiser list allergies, with a consent line, so your quote and your shopping list start from the right numbers.

Tips for this form
- Rename the food styles after your own packages, so each option matches something on your price list.
- Keep the address required: the distance decides your delivery cost and whether you take the job at all.
- Ask for the final guest count and dietary numbers a few days before the event, and say in your quote by when you need them.
- If you have a minimum order, put it in the form description so small requests don't surprise you or the client.
Questions about this template
Why ask about tableware and tables?
Because hiring or bringing them changes the price and the van you need. Some venues have their own; offices and private homes often don't.
Why count vegetarian and vegan guests separately from allergies?
Because one is a quantity for your planning and the other is a safety note for the kitchen. The number of vegetarian guests goes straight into your quote; allergies need a closer look and a conversation with the organiser.
Can I use this form for regular office lunches?
Yes. Add a question about how often (one-off, weekly, monthly) and the form works for recurring orders too.
