Private dining and venue hire request form

A table booking needs a time and a number. A private event needs much more, and it usually arrives as a phone call in the middle of service. This form takes the details you need to make an offer: the occasion, the date and time of day, how many guests, whether they want a separate room or the whole venue, the menu format, a budget per person and extras such as a microphone, a screen or their own cake. Allergies are asked once for the whole group, with a consent line, so the kitchen knows early. You reply with a proposal instead of a list of questions.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Name your rooms or areas in the space question, with the number of guests each one seats, so guests can choose for themselves.
  • If you charge a minimum spend or a room fee for exclusive hire, say so in the form description.
  • Offer the own-cake and own-decoration options only if you allow them, and mention any cake-cutting fee.
  • Keep the guest count required; it decides which room you can offer before anything else.

Questions about this template

How is this different from a table reservation form?

A reservation form books a table at a time. This form is for events that need an offer first: a set menu, a separate room, exclusive hire or extras. You can use both side by side on your website.

Why ask for a budget per person?

It tells you straight away which menu to propose. The field is optional and free text, so guests can write a range or leave it empty.

Why is there a consent line for allergies?

Allergies and dietary needs can count as health information. The line records that the person organising the event agrees to these details being stored and used to plan the food.
