Volunteer sign-up form
Collect volunteers' interests, availability, skills and an emergency contact, with parental agreement for under-18s.
Many small associations get offers of help in passing, at an event or in a message, and lose track of them before anyone follows up. This form records each pledge in one place: who the donor is, whether they give privately or for a company, what they'd like to give, roughly how much and for which cause, and whether you may name them as a supporter. It takes no money. A note on the form says that you'll reply with your official ways to give, so donations always reach the account your board controls.
