Tips for this form

Can I see how each person wants to be contacted?

Yes. The last question asks whether they prefer a call, an email or a message, and the answer is included in the email you receive.

Where are enquiries stored?

Each enquiry is emailed to your account address and saved under Submissions in your dashboard. You can export them as a CSV file at any time.

Can I add questions about health or experience?

Yes, in the builder. Ask only what you need before the first session, and if you collect health details, add a consent checkbox like the one in the trial class template.
