Contact form for small businesses

A contact form only helps if people actually send it. This one stays short: name, what the message is about, the message itself and how the person would like an answer — by phone, email or WhatsApp. The topic dropdown sorts messages before you open them, so a question about an existing order doesn't get lost between new enquiries. Email and phone are optional; change that if you can only reply one way.

Tips for this form

  • Rename the topics after what people really write to you about; four or five options are enough.
  • Put the form link wherever people look for you: your website's contact page, your Instagram bio and your Google Business Profile.
  • If you only reply by email, make the email field required, so no message arrives without a way to answer it.
  • Turn on the auto-responder and say in it when you usually reply, for example within one working day.

Questions about this template

Why use a contact form instead of just showing my email address?

A form asks for the details you need in one go, and every message lands in the same place: in your inbox and under Submissions in your dashboard. You can still show your email address and phone number next to the form for people who prefer them.

What is the topic field for?

It tells you at a glance whether a message is a new enquiry, a question about an existing order or something else. It also makes it easy to answer order questions first, before people start to wonder where their order is.

Do visitors have to give a phone number?

No. In this template only the name, topic and message are required. Ask for as little as you need: every extra required field is one more reason to give up halfway.
