Contact form for small businesses
A short contact form that also asks what the message is about, so you can answer the right question the first time.
This survey is built around the Net Promoter Score question: how likely is the customer, from 0 to 10, to recommend you to a friend or colleague? Because everyone answers the same question, you can compare results from one round to the next. The useful part is in the follow-up questions: the main reason for the score, what you already do well, and the one thing the customer would change. Unlike a feedback form sent after a single visit, it asks about the customer's experience with your business as a whole, so it works for shops, studios, workshops and service businesses alike.

Count answers of 9 or 10 as promoters and answers from 0 to 6 as detractors; 7 and 8 are passives. The score is the percentage of promoters minus the percentage of detractors, so it lies between −100 and +100. Zotview doesn't calculate it for you: export the answers and count them in a spreadsheet.
Better not. Ask every customer for a review separately: Google's guidelines don't allow asking only satisfied customers for reviews, and a survey that sorts people by score first would do exactly that. To collect quotes you may publish, use the testimonial request template instead.
A feedback form asks about one appointment or order while it's fresh. This survey asks about the relationship as a whole and works best sent to many customers at the same time, so you can compare one round with the next.
A short contact form that also asks what the message is about, so you can answer the right question the first time.
Customers ask for a time with a first and second choice of date, so you can usually confirm with one reply.
Collect customer testimonials with a rating, the customer's own words and clear permission to publish them.
The template opens in the form builder, where you can edit every question and choose who receives the submissions.
