Contact form for small businesses
A short contact form that also asks what the message is about, so you can answer the right question the first time.
Happy customers are usually glad to say something nice, but few write it down unprompted, and you can't put a private WhatsApp message on your website without asking. This form asks for an overall star rating, what the customer bought or booked, their experience in a few sentences and how their name should appear: in full, as first name and initial, or not at all. A checkbox records whether they allow you to publish it, so you know which quotes you may use on your website, in social posts or on a flyer.
Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.
