Repair request form

Repair shops lose time on the phone working out what a customer has, what's wrong with it and how it will get to them. This form collects it in writing: the kind of item, the brand and model, a description of the fault, whether it's still under warranty, and whether the customer will bring it in, wants it collected or needs the repair where it is. An optional cost limit tells you up to which amount you may go ahead without calling first. It suits phone and computer repairs, appliance and bike workshops, and small garages alike.

Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Tips for this form

  • Shorten the item list to what you repair; a bike workshop doesn't need the car option.
  • If you run a garage, rename brand and model to make, model and year, and add a field for the registration number.
  • Say in the form description whether you charge for diagnosing the fault, so customers know before they hand the item over.
  • If the cost limit is too low for the likely repair, get in touch before you start the work.

Questions about this template

Why ask about the warranty?

A repair under the manufacturer's warranty or the seller's guarantee is often handled by them, and opening the item yourself may affect that cover. Asking first lets you point the customer the right way before you take the job.

What is the cost limit for?

It's the amount up to which the customer agrees to the repair without a call. Below it you can go ahead; above it you get in touch first. The field is optional, and the agreement itself is between you and your customer.

Is a submitted request already a repair order?

No. The form only tells you about the job. When and on which terms you accept it is up to you, for example after you've looked at the item and given an estimate.
