Salon appointment request form
Clients request a hair, nail or beauty appointment with their service, preferred day and time of day.
In a barbershop or salon, a CV says less than a few photos of someone's fades or colour work. This form is written for chair staff: the role (barber, hairstylist, colourist, nail or beauty, assistant or reception), years of experience, the skills they're strongest in, whether they're qualified or still training, full-time, part-time, weekends or chair rental, which days they can work and when they could start. An Instagram or portfolio link replaces the CV, and a note asks applicants not to send ID or residence documents with the form.
Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

Zotview for BarbershopsBecause for chair staff, recent work shows more than a CV, and a link is easy to send from a phone. If you need a CV, ask the people you shortlist to email it.
ID, passport and residence papers aren't needed to decide who to invite, and sending them through a web form isn't wise. Ask for what the law requires when you make an offer.
Yes, renting a chair is one of the working patterns. Say in the description what the rent covers, so you only hear from people for whom the deal works.
Clients request a hair, nail or beauty appointment with their service, preferred day and time of day.
Guests choose treatments, group size and a preferred date, so you can reply with a fitting offer.
Collect skin type, goals and known sensitivities before a facial, peel or laser appointment.
Let regulars ask to join your monthly barber plan with the plan, frequency, barber and start date they want.
Know a client's hair history, the result they want and any past reactions before the colour appointment.
Record treatments, health questions, patch test status and the client's typed confirmation before lash or brow work.
The template opens in the form builder, where you can edit every question and choose who receives the responses.
