Contact form for small businesses
A short contact form that also asks what the message is about, so you can answer the right question the first time.
In a small team, holiday requests come in by text, on a note by the till or in passing, and two people end up off in the same week. This form puts every request in one list: who is asking, what kind of leave it is, the first and last day, whether a day is only half, and who will cover. It's for leave planned ahead, not for reporting sickness, so it asks nothing about health; the note field reminds people not to share health details. You reply to confirm, and the submission list shows at a glance who is away when.
Opens as an editable form in your dashboard. If you're not signed in yet, you'll sign in or create an account first.

No. Each request reaches you by email and is saved under Submissions; you confirm or decline it yourself, for example by replying to the employee.
It isn't meant for that. Health details are sensitive data, so the form only covers leave planned ahead; sickness is reported the way the rules that apply to you say.
No. The form collects requests; count remaining days in your payroll or a spreadsheet, using the CSV export of the submissions.
